Because every UHC plan is administered under a different name, the cost can vary. While exchanging your health care information to our staff, make sure to mention if there are other company names on the card, this will help our staff to verify your benefits correctly.
All plans under the UHC umbrella are subject to the United Health Care fee schedule, which has a maximum amount for each visit- not individual services. US health group policy holders typically have fewer visits allowed, but they are covered at 100%.
If your card only reads United Health care across the top, you are part of their main network. There are certain plans that require a clinical submission, see more information below. This is to ensure that the visits are being used for pain management and not maintenance care.

Clinical submission is a form that the patient is required to fill out at the time of service at different times during their treatment plan. These forms ask various questions about your current episode of pain. It usually takes about 5 minutes to fill out these forms.
They are sent off by our staff and reviewed by a Doctor in the Pain management department at UHC. When they are finalized by UHC, there is a determination letter that will give a set date range/visit limit for that period. Once the visit limit has been reached or end date has passed, another clinical submission will be required.
These limitations are based on the 6-character group number on the front of your insurance card. Having your insurance card in hand will help our staff gather the appropriate documents needed at the time of your visit.
Typically, chiropractic care does not require prior authorization from your insurance company or your primary care physician.
If you are concerned with the specific details of your benefits, there are a few options. You can always contact your insurance company and speak to a member representative; they will walk through various services and your policy limitations and maximums.
If you have made your appointment at least 72 hours in advance with our office staff, you can request to have these details given to you prior to your initial visit after we have verified them.
